Pharmacological properties
Kexili-0.1 and Kexili-0.3, combines two active ingredients: Adapalene and Benzoyl Peroxide, which act by different, but complementary, mechanisms of action.
Adapalene is a chemically stable derivative of naphthoic acid with activity similar to that of retinoids. Studies of the biochemical and pharmacological profile have shown that adapalene acts in the pathology of Acne vulgaris, since it is a powerful modulator of cell differentiation and keratinization and has anti-inflammatory properties.
Adapalene binds, like tretinoin, to specific nuclear receptors of retinoic acid, but unlike that it does not bind to cytosolic receptors. Adapalene administered topically is comedolytic and also has effects on the abnormal processes of keratinization and epidermal differentiation, both of which are present in the pathogenesis of acne vulgaris. The mechanism of action of adapalene may be a normalization of the differentiation of follicular epithelial cells, resulting in a decrease in the formation of microcomedones. In standard anti-inflammatory assays, adapalene is superior to reference retinoids, both in vivo and in vitro. Pharmacodynamically, it inhibits the chemotactic and chemokinetic responses of human polymorphonuclear leukocytes and the metabolism by lipoxidation of arachidonic acid to proinflammatory mediators. This profile suggests that the mediating cells of the inflammatory component of acne may be modified by adapalene. Human studies provide clinical evidence that Adapalene is used topically to reduce the inflammatory component of acne (papules and pustules).
The absorption of Adapalene through human skin is low, and therefore interaction with systemic-acting medicinal products is unlikely. There is no evidence that the efficacy of oral drugs such as contraceptives and antibiotics is influenced by the cutaneous use of products containing Adapalene.
Benzoyl Peroxide kills acne-causing bacteria, dries out breakouts, aids in skin peeling, and reduces the number of comedones (pimples) that block pores. It also has a mild drying effect, destroying excess oils, bacteria and dirt that clogs above and below the skin, and has no effect on sebum production or the spread of dead skin cells. The mechanism of action of Benzoyl Peroxide is unknown, but it is known that it is broken down in the skin by cysteine, releasing oxygen free radicals that oxidize bacterial proteins in the sebaceous follicle, thus decreasing the number of bacteria and irritation caused by fatty acids.
Its effectiveness in the treatment of acne vulgaris is mainly attributed to its antibacterial activity on propionibacterium acnes, the predominant bacterial agent in sebaceous follicles and comedones of acne skin.
5% is absorbed through the skin, where it is metabolized to benzoic acid and then excreted as benzoate in the urine. Benzoyl peroxide is a known anti-acne active ingredient. Its activity in the symptomatic treatment of acne vulgaris is antibacterial, keratolytic, comedolytic and desiccant in the affected area. In general, a visible improvement in the symptoms of acne vulgaris can be observed five days after treatment.
Therapeutic indications
Kexili-0.1 and Kexili-0.3 are indicated for the topical treatment of acne vulgaris and moderate to severe papulopustular inflammatory acne in patients 12 years of age and older.

How to use
Wash the affected part (face, back, chest) once or twice a day with the Kexili anti-acne bar, using as little water as possible and pressing gently to foam, leave Kexili applied for a few minutes and rinse to remove. Do not use another cleaning soap in the area under treatment because it will cancel the therapeutic action sought. Avoid contact with eyes, lips and nostrils. At the beginning of the treatment it is normal to momentarily feel a slight sensation of heat or itching. Benzoyl Peroxide can whiten hair or fabrics so you should avoid contact with them.
Drug interactions
Do not use Kexili while you are using tretinoin, using these products together could cause severe irritation.
Kexili must not be combined with other scaling treatments, preparations containing alcohol, abrasive cleansers, desiccant cosmetics or other topical medications, unless medically indicated.
Adapalene can potentially produce mild local irritation, so it is likely that combined use with exfoliating agents, astringents, or irritating products, may cause additional irritating effects.
Side effects
Skin dryness, contact dermatitis, burning and skin irritation and feeling of heat at the application site.
The most frequent adverse reaction that can occur is skin irritation, which is reversible by reducing the frequency of application or by discontinuing treatment.
If the patient has a skin reaction, the effects usually wear off within 48 hours after stopping Kexili use. If the symptoms persist, it could mean that the skin is not tolerant to Benzoyl Peroxide and you should not use products that contain it in its base.
Cautions
While using Kexili you should avoid the use of irritating skin products, such as strong soaps, shampoo or skin cleansers, hair coloring or chemicals for perms, depilatories or waxes, alcoholic skin products, spices, astringent, or lime. Do not use other skin medications unless your doctor tells you to. Avoid using sunscreen-blocking creams that contain PABA, as skin color changes can occur.
Kexili is for external use only and contact with the eyes, lips and nostrils should be avoided. If these regions come into contact with the product, they should be washed with plenty of water. Do not use Kexili on sunburned skin regions, dry, red, irritated skin or open skin. Also avoid the use of Kexili in wounds and regions with eczema. Wait until these conditions are cured before using it.
Seek emergency medical attention if you develop any of these symptoms of an allergic reaction: hives, shortness of breath, swelling of the face, lips, tongue or throat or if you have any symptoms not described in this leaflet. Stop using Kexili and see a doctor immediately if you have severe itching or burning on the skin after using Kexili.
There is not information about the effects of Adapalene in pregnant women and therefore Kexili should not be used during pregnancy unless it is considered essential by the doctor, due to the risk of teratogenicity seen in animal studies. It is not known whether Adapalene is excreted in human or animal milk; therefore, the use of Adapalene in lactating women should be avoided, but when using it and to avoid contact exposure of the newborn, it should not be used. You must apply the product to the woman’s chest. Although various animal and human studies have shown that Adapalene lacks phototoxic and photoallergic potential, the safety of use of Adapalene has not been established during repeated exposure to sunlight or UV radiation in animals and man. Therefore, excessive exposure to sunlight and UV radiation should be avoided during treatment with Adapalene. The absorption of Adapalene through human skin is low, so interaction with systemic medications is unlikely.

Conservation

Contraindications
Kexili anti-acne bar is contraindicated in patients allergic (hypersensitive) to Adapalene, Benzoyl Peroxide or any of its components. Kexili should also not be used if you have very sensitive or irritated skin.
