Properties

Vio-Geric (donepezil hydrochloride) is a reversible inhibitor of the enzyme acetylcholinesterase.

Therapeutic indication

Vio-Geric is indicated as an auxiliary for the symptomatic treatment of mild to moderately severe dementia of the Alzheimer type.

Pharmacokinetics and pharmacodynamics

Donepezil hydrochloride exerts its therapeutic effect by increasing cholinergic function in the central nervous system. This is accomplished by increasing the concentration of acetylcholine by reversibly inhibiting its hydrolysis by acetylcholinesterase, one of the enzymes that hydrolyze it.

Donepezil is well absorbed, with 100% relative oral bioavailability, and reaches peak plasma concentrations within 3-4 hours. Its pharmacokinetics are linear in the dose range of 1-10 mg administered once a day. Neither food nor the time of administration (morning or evening) influence the rate or magnitude of its absorption. The elimination half-life of donepezil is almost 70 hours, and its apparent average plasma clearance is 0.13 L / h / kg. After administration of multiple doses, donepezil accumulates in the plasma on the order of 4-7 times and reaches steady state in no more than 15 days.

Donepezil is excreted in the urine, both unchanged and extensively metabolized into four major metabolites, two of which are known to be active, in addition to several minor metabolites, all of which are not identified.

Contraindications

Vio-Geric is contraindicated in patients with known hypersensitivity to donepezil hydrochloride or to piperidine derivatives.

Side effects

The most frequently reported adverse reactions are: nausea, diarrhea, vomiting, fatigue, insomnia, muscle cramps and anorexia.

Administration route

Oral.


Posology

 

Doses of 5 to 10 mg are clinically effective. Treatment should be started with 5 mg / day and may increase to 10 mg after 4 to 6 weeks.

The maximum recommended daily dose is 10 mg. Doses greater than 10 mg / day have not been studied in large-scale clinical studies.
